Challenge Forms

The form provided below is an adaptation of the recommended form put forth by the ALA. SJCPL keeps forms on hand at all points of public contact, including circulation, reference, children’s services, and in the offices of branch managers. Please become familiar with the form and where it is kept in the library so that we may assist a patron if they choose to request reconsideration of material.

Request for Reconsideration of Library Resources

The board of Mainstream County, U.S.A., has delegated the responsibility for selection and evaluation of library resources to the collection development team, and has established reconsideration procedures to address concerns about those resources. Completion of this form is the first step in those procedures. If you wish to request reconsideration of library resources, please return the completed form to the Library Director, Donald J.
